rental contract

Does anyone have a source for a boilerplate rental contract that you've used with your Red package? (Or really, any similar gear.) Been doing a few rentals on the side, and I'd like to have some legalese for people to sign when taking my camera out. Nothing fancy - most of these folks are friends/associates - just something to make it "official"... maybe including language about insurance, liability, etc. (State of New York here, fyi.)
